Serena agents aren't actually referencing MCP tools properly

I tried to show a friend one of our serena agents last night - duplicate code detector - but noticed tools were missing:

The activate_project and create_issue tools are mentioned but not defined or available, so I'll note their absence and instead proceed with a textual report since I cannot invoke these tools or request approval. I'll start by inspecting the git status to analyze recent changes.

Working on being back serena only . To get the olde behavior, enable "model: local"
